PATIENT ACCESS ON-LINE
You can book appointments; request repeat prescriptions; change personal details; and view your medical record on-line. To do this you must first bring a driving licence or passport to the Surgery; ask the Receptionist for details of how you register for Patient Access;receive a Registration letter. Once this has been obtained you can register to obtain these services by clicking below on the "Register for online services" button or clicking on the relevant icon below that button.
FRIENDS AND FAMILY TEST
Every Practice is now required to seek feedback on whether or not patients are likely to recommend the Practice to friends and family if they needed similar care or treatment. You may be asked when you attend the Practice to complete a simple questionnaire. However you can also complete this by clicking on the "Take our questionnaire button" below.
NAMED ACCOUNTABLE GENERAL PRACTITIONER
From 1st April 2015 Practices are required to allocate a named accountable GP to all patients- including children. The named accountable GP for all patients at this Practice is Dr G C Sinha who is responsible for their overall care. He has responsibility for the coordination of all services under the GP contract.

CARE QUALITY COMMISSION
The Care Quality Commission is inspecting the Practice on Thursday 20th September 2016. If you have any comments to make about the Practice please ask a Receptionist for a comment card or if you wish to speak to an Inspector in confidence contact the Prcatice Manager who will arrange this.
